Abstract

Nowdays, microvascular flaps are usually employed for the head and neck defects reconstruction at the majority of the Maxillofacial Departments, however, the actual trend is to use a few number of flaps. In those cases that combine a large soft tissue defect and segmental mandibular resection, osteofasciocutaneous scapular flap provides a segment of well vascularized bone and also multiple large skin paddles and is good alternative to the use of double free flaps. this composite flap is in hour hands the treatment of choice in these cases. Four patients with head abnd neck tumors which involves soft tissue and mandible were treated at Instituto de Cirugía Maxilofacial, Madrid, fron January 1995 to June 1998. All the patients underwebnt a segmental mandibular resection and a large soft tissue resection. Primary reconstruction was performed by means an osteofasciocutaneous scapular flaps. All flaps survived. No mayor postoperative complications were observed. The aesthetic and functional results were satisfactory.
