Resumo

Objective: To evaluate the clinical and radiological results of patella thinning osteotomy in the management of long-evolving isolated patellofemoral osteoarthritis. Material and methods: Thirty-one patients (35 cases) were treated since 1991 with a new patellar thinning coronal osteotomy. The mean age at surgery was 61 years (range 44-77), with a patella thinning of 5 mm and a mean postoperative follow-up of 6 years (range 1-15). Evaluation of the clinical outcome was based on the Knee Society Score and the Patellar Score, with the SF-36 questionnaire for patient satisfaction. A number of radiographic parameters were also evaluated to determine the articular interline and the progression of osteoarthritis. Results: Improvement was observed on the functional scales, patient satisfaction and the radiographic parameters � with favorable results in terms of the articular interline, angles of congruence and tilting, and stabilization of the radiological progression of patellofemoral osteoarthritis. Although progression of the femorotibial gonarthrosis was recorded in 50% of the cases, a total knee prosthesis only proved necessary in three cases, with no technical implantation problems. Conclusion: We present a new technique affording improved results in the management of isolated patellofemoral osteoarthritis, without having to sacrifice the patella
