Abstract
Aim Among odontogenic abscesses, the periodontal abscess is specially relevant, due to its influence on tooth prognosis, its relation with periodontitis, and the possibilities of the spreading of the infection. However, information is scarce and with low scientific quality. Summary. Based on recently published knowledge, an alternative classification may be proposed, to help in the prognosis and treatment planning. Periodontal abscesses can be related to a pre-existing periodontal pocket, or they can occur without any link with periodontitis, both related to impacted foreign objects, or to alteration or damage in the radicular morphology. A suitable characterization of periodontitis periodontal abscess is also proposed, from both clinical and microbiological points of view. The treatment of periodontal abscesses is specially relevant, and its management is discussed, focusing on periodontitis periodontal abscesses. The importance of drainage and debridement, and the possible role of systemic antimicrobials, is analyzed following the available scientific data. Conclusion. Although the need of new research on this subject is obvious, the available data suggest the importance of drainage and debridement as a fundamental step in the management of this condition, and which are the most relevant factors in the selection of the right antimicrobial and its dosage, if necessary.
