Abstract
The clinical history is a fundamental document in medical assistance. Clinical history in odontology takes a very special relevance in clinical risk management and in lawsuits against dentists because it gathers the conditions in which the different assistance procedures have been taken place. Because the leak of records about intraoperatory facts that support the professional intervention, an ‘oral surgery operatory sheet’ and an ‘oral implantology operatory sheet’ are proposed. Both documents will include two requirements that are established in law 41/2002 article 15.2 as mandatory for surgery procedures with hospitalization: anaesthetic report and operating theatre report, allowing the detail of the conditions of surgery activities, the materials used and would ease an ordered and reliable record to contribute in a trial in case of a lawsuit.
