Basaloid carcinoma is a low frequency squamous cell carcinoma variant. This type of neoplasm is characterized by its distinctive histology, aggressive biologic behavior and its predilection for the upper aerodigestive tract. Due to its aggressive behavior, early cervical or distant metastases are a commonfinding, even at the time of diagnosis. The most interesting feature of this tumor may be its histology, with a mixed component of basaloid and squamous cells that may lead to confusion with other neoplasms, like adenoid cystic carcinoma or classic squamous cell carcinoma. We include a case of basaloid squamous cell carcinoma affecting the tangue base and the floor of the mouth and extending into the pharinx, as knowledge of this infrequent tumor variant is interesting for both treatment and prognosis.
