Estudio clínico-patológico de la profundidad de las bolsas periodontales en relación con la cualidad del infiltrado inflamatorio

Abstract

158 patients with advanced chronic periodontitis (PDI>4 ), aged between 30 and 65, have been studied in order to find an histologic inflammatory parameter related to advanced disease. The patients had been selected previously under radiological and clinical criteria (periodontal probing). After grouping the samples in type I pockets: (4-6 mm) and type II (6-12 mm) a biopsy of these pockets was taken. The infiltrates found were grouped into five types attending to the predominant cells observed and statistically correlated to the different probing depths. The mixed inflammatory infiltrated composed by plasma cell ad lymphocytes (type C, 37,9%) was the most frecuent and related to pockets tipe l. The type D infitrate (plasma cells and PMN) only appeared in deep pockets. Statistically significant differences (p<0,01) were found among deep and shallow pockets, attending to their inflammatory infiltrate. The predominant is the plasma cell which increases in the deepest pockets. Since its presence is independent from activity, it cannot be used as an histological marker of it. Therefore, new histological markers should be designed to value the disease activity.
