Abstract

Agenesis of one or more teeth is the most common anomaly of dental development in man. According to evolutionary theories, agenesis trends to increase and thereby dentofacial alterations that require specific treatment. The aim of this article is to demonstrate an uncommon manifestation: the absence of a first permanent upper molar and the possible dental complications. The lack of eruption of the permanent molar had consequences in the maxilla and mandible. The dental approach, consisted in eliminating the distal surface of the upper primary second molar locking the spontaneous eruption of the inferior permanent molar. The upper primary molar suffered supraeruption because the permanent molar germ was not present and could not stop the distal displacement of the roots, as should be done by the germ of the permanent molar. The processes of eruption and occlusion development are submitted to multiple alterations. Therefore, the need of frequent check-ups of the patient is essential, this will allow early diagnosis and timely therapeutic intervention to prevent disturbance.
