Concept The skin as a barrier organ is exposed to various types of physical aggression from the external environment. This aggression can be mechanical or caused by heat or radiation (ultraviolet and ionising). Aetiopathogenesis Intense cold causes progressive ischaemia in the skin, burns cause progressive thrombosis of the blood vessels and radiation damages DNA. Classification The main forms of dermatosis caused by physical agents are burns, freeze injuries and radiodermatitis. Diagnosis A detailed history is essential to diagnose the condition and direct us to the causative agent, establish the conditions and the time the accident took place, as well as the circumstances that the patient was exposed to, and any comorbidity that might aggravate the clinical picture. A physical examination is required to assess the general condition of the patient. Prognosis The prognosis will depend on the extent of the dermatosis. Treatment Burns are treated by cooling, cleaning, debridement and creating a moist environment for re-epithelialisation. Freeze injuries should be rapidly warmed with water at 37°-39° for about 15 to 30 minutes, and topical, medium/high potency corticosteroids can be used for radiodermatitis, if there is no ulceration and/or superinfection.
