Types Skin infections are increasing in frequency and severity and can be classified into 3 large groups: bacterial, fungal and viral. Treatment Most bacterial infections, of typical clinical presentation, can be treated empirically. If there is doubt as to the diagnosis, a sample of the lesion is obtained for gram stain and culture. Necrotising fasciitis is an exception because if suspected, an exploratory fasciotomy and culture of exudate are compulsory. Diagnosis A diagnosis of herpes simplex virus infection is usually clinical, but if there is any doubt a culture or PCR should be obtained. Human papilloma virus infections are also usually diagnosed clinically, and biopsies and histological tests performed if in doubt. Direct examination is recommended with fungal skin infections, visualisation rapidly confirms the fungal structure, and then a culture should be obtained to confirm the species.
