Abstract

The continuity in the management of care allows, in an organized way, to direct the attention to the people with health issues to provide human and quality attention. The objective of this study is to determine the impact of the actions and follow-up carried out by the nursing network, since hospital discharge, and the re-admissions of risk users of the Eastern Network in Costa Rica. Correlation study, ex post facto, performed between May 2015 and September 2016. Sociodemographic, follow-up and cost-related data were collected through a questionnaire validated by experts. The main results show that despite the efforts made by nursing professionals to carry out an adequate approach from the continuity of care approach, the initiatives shown are very incipient and do not allow to affirm that there is a continuous follow-up or a protocolized management And controlled cases, so it is necessary to reorganize the follow-up activities carried out by Nursing in the Eastern Network of the country. To achieve a care management model, as described by the international referents, it will be necessary to invest in nursing human resources and training in skills of advanced nursing practice; As well as to continue to investigate the variables that influence re-entry and related costs in Costa Rica.
