Abstract

BACKGROUND: There is a high component of medical variability which turns the hip arthroplasty into a process with different results as to the length of stay. OBJECTIVE: to evaluate the length of stay in hip arthroplasty procedures. PATIENTS AND METHODS: A mixed methodology was performed. We have analyzed organizational, scientist-technical and outcome indicators in the professionals’ and patients’ discourse of the orthopedic department with qualitative research. We have analyzed patients´ histories within DRG 818 for 2008. RESULTS: The age (average: 77.36 years, SD:12.38, range 40-95), hip fracture with hemiarthroplasty (61.4% of patients), total inter-medical reports (average: 2.54,SD: 2.536, range 0-12) and the delay to deal with medical reports for the rehabilitation department (average:2.53 days, SD: 3.166, range 0-16), are linked with an increase length of stay. These results match with those obtained on the qualitative research. CONCLUSIONS: The variability found seems to be caused by the kind of surgery, not using clinical pathways or teamwork, not planning the discharge and not managing properly beds and social problems.
