Abstract

The technique used to block the superior maxillary nerve (transpalatine route or pterygopalatine canal approach) is revised here, begining from its origin, indications, technique and most frecuent complications. With this technique using the greater palatine foramen and going through the pterygopalatine conduct is reached the pterygopalatine fossa just where the second branch of the trigeminal nerve emerges of the skull. This is a relatively easy technique, considering the mean angulation of the needle, 45°, and the deep average, 35 mm. The pterygopalatine canal has obstruction in 6% of the patientes. The clinical efficiency is about 85% in molars and premolars (mainly in extractions). The visual alterations are the principal complications and are present in the 1-39% of the cases.
