Résumé

We report the case of the 60 years old female corpse, deceased 15 years ago in Madrid. It shows limited lesions in the left os coxae and the skull. In the latter highlights a lesion in “punch” in the right anterior frontoparietal area, 2 cm in diameter, with marked circularity, whose superior edge reach the frontotemporal line. In the right os coxae there is a similar lesion, discreetly oval with a maximum diameter of 25 mm and minimum of 18mm in the outer surface (lower on the interior surface), located in the illium, near the posterior iliac spine over the retroauricular region. In the radiological study is not observed a reactive sclerotic reaction in none of the lesions. They are rated as possible diagnoses the multiple myeloma, eosinophilic granuloma (histiocytosis X) and bone metastases. Considering the “punch” morphology, the dimensions, the number of lesions, their location, age and the radiological images, it is considered as the most probable diagnose a multiple myeloma.
