Abstract

A significant percentage of medication errors occur during transitions of care and can affect both the effectiveness of treatments and patient safety. The aim of medication reconciliation is to ensure that the patient receives the medications he was taking prior to a care transition, in the correct dosage and regimen and in accordance with his new clinical situation, thus reducing errors.
