Inferior oblique muscle palsy (IOP) is an extremely infrequent condition and the diagnosis can be difficult. Data from five patients with IOP, congenital (2) and acquired (3) are compared. All five patients presented limitation of supraversion in adduction, which improved with the corresponding duction, diplopia (3), hypotropia in primary position (4), intorsion (4), A-pattern (1), and torticollis (4). Two underwent surgery with improvement of the symptoms. IOP should be taken into account as a differential diagnosis in patients with vertical diplopia and limitation of supraversion in adduction. Surgical cases should be carefully selected
