Zusammenfassung

Introduction: Anomalous orbital structures are rare causes of restrictive strabismus. One involves accessory muscle fibers originating in the posterior orbit and inserting on the globe, optic nerve, or extraocular muscles. Clinical case: We described a child who were diagnosed and operated on as congenital third nerve palsy with a normal magnetic resonance report. However, because of the severe restrictive and fibrotic muscles that rendered surgery impossible, a second magnetic resonance imaging revealed an anomalous unilateral orbital structure that limited ocular movements. Conclusion: Computed tomography and, particularly, orbital magnetic resonance imaging, are essential for correct diagnosis of severe and anomalous strabismus.
