Resumo

Introduction. The implantation of cardiac devices has increased exponentially in recent years, highlighting the need for a nurse to assess not only the device but also the patient‘s needs. Objective. To describe the care plan for an 84-year-old woman with an epicardial pacemaker, whose pulse generator had migrated from its original subxiphoid position to the vicinity of the Douglas cul-de-sac. Methods. A nursing assessment was carried out using the Marjory Gordon model of functional patterns and a nursing plan was developed using NANDA-NOC-NIC taxonomy.Results. The patient presented the following nursing diagnoses: anxiety, poor knowledge, risk of infection, and risk of reduced cardiac output. The risk of visceral injury due to generator migration was also identified as a collaborative problem.Discussion and conclusions. The patient received information about alarm symptoms and the behavior to follow, which reduced her anxiety until the time of implantation of a new endocavitary lead and generator in the left pectoral. A small incision was made to cut the old lead. With the description of this clinical case, the complexity and role of the nurse in device consultations are exemplified.
